Getting Started Information Page.
Thank you for choosing BPM to provide for your
Bookkeeping and Tax Reporting compliance needs.
To enable us to setup your business books, tax
compliance cycle and begin providing the services, we
will need some information from you that will form part
of our files at the getting started stage and going
forward.
1st., The Tax organizer.

The purpose of this form is to help you zero into and
easily pull together all of the applicable business
documents, information and expense receipts needed for
us to do the required bookkeeping and applicable year
end business and personal tax returns.
2nd., The Start-up Information Sheet.

Please complete this form with all applicable
information
3rd., The Business Consent Form.

Enter your business name in part 1, your telephone
number, the business number and complete and sign the
part 5, section of the form.
This form will inform CRA that we have been retained to
represent you in matters relating to the business Tax
returns including GST returns and information.
4th., The personal tax Authorization Form

Complete the Part 1 section of this form with name and
social insurance number, and sign the signature section
in Part 6. This form is used to represent you on
personal tax matters with CRA.
Keep in mind that included in our service, we also file
the personal tax of your spouse, so please print a copy
to be completed by your spouse.
5th., The personal Tax Information form.

This form is used to provide us with the personal
information required for the preparation of your tax
return. Complete the first section and the dependent
section. Leave the T-SLIPs and the other sections blank
unless you will be providing T-Slips applicable to your
year-end tax return.
If you have and will be providing us with T-Slips,
indicate the number of each in the space provided and
send us the originals. If preferred, you may send us
copies and bring the originals with you to our office at
the time scheduled for you to sign the returns.
